Here are the links to some commonly used request forms.
If you cannot find a referral form you are looking for please see our useful documents section
HEA/HLA typing
This form is used to test both human erythrocyte antigen (HEA) typing (56 antigens) and HLA typing through the genotyping programme. This testing is done as part of a piece of work which will hopefully allow for better matching of donated blood in the future. See this patient information leaflet for more information.
HLA typing of relatives for potential allogeneic stem cell transplant
This form is used for tissue typing of relatives when assessing whether a patient has a matched donor.
Genotyping of red cell disorders
This is used to confirm the underlying disease genotype of the patient (e.g. number of mutations or identifying the specific genetic mutations in alpha/beta thalassaemia). Can be useful to diagnose haemoglobinopathies in transfused patients.
